VisionTree®, a leader in cloud-based, patient-reported outcomes, patient engagement, population health and multi-center research/registry solutions, is pleased to announce that the VisionTree Optimal Care™ (VTOC) platform has been selected by Americares, Loyola University and the National Association of Free & Charitable Clinics for its Roadmap to Health Equity initiative. VTOC will be used as a national data repository to collect patient-level outcomes and process quality measures data across an estimated 1,400 free and charitable clinics ("FCC") nationwide. Collectively, the FCC network provides approximately 5 million healthcare visits for nearly 2 million patients with a support of 8 million volunteer hours each year.
The health equity initiative is a joint project of Americares, the National Association of Free & Charitable Clinics and Loyola University Chicago in collaboration with state clinic associations and clinic leaders across the country. The multi-year initiative aims to track patient outcomes and measure quality of care at free and charitable in the U.S. The goal of the repository is to enable clinics to increase capacity, improve health outcomes, build academic evidence and strengthen advocacy for funding and community support.
The proposed data repository will contain data on patient and clinic demographics as well as patient-level health outcomes. Initially, clinics will report up to fifteen (15) quality measures and five (5) sociodemographic factors. It is envisioned that clinics utilizing an EMR with reporting capacities may submit whole patient population data, while those clinics that are paper based or using a lower-capacity EMR will submit data from a randomized set of 70 patients through mandated evidenced-based sampling methodologies. Participation in reporting quality measures is voluntary.
The initiative's larger purpose is to provide FCCs with the means and opportunity to enhance their quality of care in their pursuit of health equity. Participating clinics will be able to view their data using VTOC's V2 for Value-Based Care on FHIR reporting tools. Clinics can also utilize benchmarking dashboards to track their progress over time and against their peers.

VTOC's interoperability solutions includes its ePROs API suite for patient reported outcomes integrated with existing patient portals supporting HL7 and Web-service interfaces for quality data import from EHR/OIS and all clinical systems.
