VTOC Outcomes App is Now Available in the Epic App Orchard

Visiontree Global Health Cloud announced today that their Visiontree Optimal Care™ (VTOC) app has launched in the Epic App Orchard. The VTOC app is designed for organizations seeking an integrated method to collect and report patient-reported and clinical outcomes for quality, research, and registry initiatives.

VTOC’s modular, enterprise-level platform deliver outcomes-driven pathways meeting NCCN, AJRR, NCQA, ICHOM, ERAS and other industry leading guidelines. The app provides a library of over 400+ validated ePRO or eCRO forms with the ability to customize your own forms for data collection. The app’s integrated ePROs Access Anywhere Suite™ with actionable alerts and V2 Package - Value-Based Health Care on FHIR® reporting, includes Rapid Reports Builder™, AWS graphical insights with predictive analytics and is proven to increase patient and medical team compliance to deliver personalized, precision medicine, as well as offer the VTOC Outcomes Center at the hospital enterprise level.

VTOC provides patient account creation via Epic HL7 ADT/SIU interfaces, which can trigger disease-site specific and research/registry templates, as well as send PDF data to Epic via an HL7 Document interface.

VTOC also supports participation in multiple studies with DataSync™. Consented patients are assigned Study ID/Case number within the clinical workflow, as de-identified data is accurately disseminated to an assigned study or registry, as well as providing visualization tools for trending longitudinal, patient-centered outcomes on patient and aggregate levels.
